While both procedures refer to the Sleeve Gastrectomy, there are some major differences between them.
Endoscopic sleeve gastrectomy is performed through the mouth and the amount of weight loss is only about 20%.
Laparoscopic sleeve gastrectomy, on the other hand, is done through the belly button and usually results in 60 to 70% of excess weight loss.

So the next question that my patients have asked me is: “Dr Choi what do you feel about endoscopic sleeve gastrectomies versus laparoscopic sleeve gastrectomies?”
So ESGs versus LSGs.
Endoscopic Sleeve Gastrectomy
ESGs or endoscopic sleeve gastrectomy is a surgery where they go through the mouth and imbricate or fold the stomach.
The weight loss is not as good as a laparoscopic sleeve gastrectomy, it’s only about 20% excess weight loss, and it does not cut out the hormone ghrelin, which is in the greater curvature of the stomach.
It is, in my opinion, an adequate surgery, but if you’re trying to lose a significant amount of weight, it is probably not the first choice.
Laparoscopic Sleeve Gastrectomy
The first choice that I would recommend is probably the laparoscopic sleeve gastrectomy, where you can lose about 60 to 70% of excess weight loss.
It does remove the hormone ghrelin which is responsible for hunger, so therefore my patients tell me, after the surgery, that they’re not hungry anymore and they don’t look for food. And it can be done laparoscopically or robotically.
A Short Review
Now just for one second, I am going to review it again, that ESG is about 20% excess weight loss versus laparoscopic surgery which is 60, 70% of weight loss.
One is done through endoscopy, through the mouth, and the other is done laparoscopically.
The laparoscopic surgery removes the ghrelin, whereas the ESG or endoscopic sleeve does not.
